Community Paramedic Program
Our Community Paramedics have additional training on top of their extensive training in the 2-year paramedic program and countless hours of continuing education. Our Community Paramedics have taken the McMaster Community Paramedicine online training course, the Premergency Community Paramedic Practitioner Program, and the Saskatchewan Polytechnic Community Paramedic advanced certificate program which requires them to complete a minimum of 210 practicum hours after 30 weeks of education.
Our community paramedic program consists of weekly wellness clinics, which are open to everyone, and scheduled at home visits for clients who meet the eligibility requirements. Our scheduled home visits are tailored to meet the needs of each client and can include things such as chronic disease management, medication compliance and management, physical assessments, vital signs monitoring, and much more.
Goals of our Community Paramedic Program
- Improve client quality of life and well-being
- Reduce client exacerbations and crisis requiring 911 emergency response
- Reduce emergency department visits and hospital admissions
- Support clients to stay healthy, safe, and independent in their own home
Eligibility for our community paramedic at home visits includes:
- Those on the wait-list for long-term care
- Those who have been assessed as eligible for long-term care
- Those who are soon to be eligible for long-term care (please see box below for long-term care home eligibility criteria)
- Those who have called EMS 3 times or more in a one month period or 6 times or more in a year
To be eligible for a long-term care home in Ontario you must meet the following criteria:
- 18 years of age or older
- Have a valid Ontario health card
- Have health care needs that cannot be met with any combination of caregiving in the home or community and
- Have health care needs that can be met in a long-term care home
